Out Patient Clinic
All Care Family Service’s Out Patient Services is intensive clinical and life skills therapy that is intended to stabilize the mental and emotional health and behavior of adults and young adults who have mental health concerns. This service is ideal for individuals who are experiencing the following: Inability to manage their behavior, emotions, and/or medication regimen in their current living or therapeutic context. Unable to manage themselves in a less structured therapeutic environment, which has resulted in the need for a more intensive level of care and treatment. Experiencing significant impairment and need mental health, behavioral intervention and stabilization to prevent psychiatric hospitalization, homelessness or possible legal involvement.
Our team of professionals includes a psychiatrist, licensed social worker (LCSW) or licensed professional counselor (LPC), social workers, qualified mental health providers, educators (QMHP) and case managers. The team of licensed professionals will conduct an individual assessment of the client’s needs and desires, to develop an individualized service plan to identify and facilitate the best outcome for the client while in ACFS care.
